Revisional Differences
In the arcade version, getting an answer wrong during the Practice simply moves on to the next question instead of ending the game. The podium's counter is also capped at 99 during Practice, making the easter egg mentioned in Night Walk Information unobtainable. Additionally, the arcade version, the Host's dialogue is automatically advanced after a few seconds without the player's input. All of these changes were reverted in Rhythm Heaven Megamix.
Usually, the OK comment is shared across all games. In Rhythm Tengoku, however, this game circumvents this by having the player get a Just OK if they fail on the 6th question in order to display a special comment. In fact, getting an OK normally is impossible. In the arcade version, failing on the 5th question also gives a Just OK. Due to Perfect being a normal Rank, getting a Superb is impossible. On the other hand, in Rhythm Heaven Megamix, it is impossible to get a Just OK. Failing on the 6th question also no longer guarantees an OK.

Unused Content
According to the debug menu of Rhythm Heaven Megamix, three trial versions of Quiz Show were planned. Although this would imply an Endless Game was to exist as well, there is no indication Quiz Special was intended to return in this game. That said, there are updated yet unused sprites of the Host's and Contestant's heads from Quiz Special, as well as the score counter.
Another remnant of this is that Quiz Show also loads the sound effect group files "GROUP_EPILOGUE_HI_LEVEL" (the Superb jingle used when clearing a trial) and "GROUP_ENDLESS" (containing sound effects typically used in gate trials), which go unused.
There is also unused text in Rhythm Tengoku: The Best+ for the Host's introduction to each trial.

Regional Differences
In Rhythm Tengoku: The Best+, the sign on top of the screen shows "QUIZ", like in Rhythm Tengoku. In the international release, it shows the Host's face and some music notes instead.
