Baikin Hakase (バイキンはかせ?, Dr. Virus) is the 4th Rhythm Game of Stage 2 (10th overall) in Rhythm Tengoku.
Gameplay
This game is uniquely set up like a tower defense game. The player controls a doctor protecting a yellow micro-organism by stabbing viruses with forks. They use ✚ to shoot the viruses from one of four directions, depending on their placement on the map.
Controls
- ✚/Tap: Shoot left, right, up, and/or down
Timing Display
- Perfect!/Ace!?: The virus is stabbed by a fork. The doctor gives a V sign and blushes. If done in succession, a star appears next to the doctor as he blushes.
- Early!/Late!?: The virus is stabbed at its edge.
- Miss...?: The virus gets through and kills the yellow organism, causing the doctor to turn blue in shock. In the Trials and the Endless Game, losing all organisms causes the game to be over, otherwise they respawn. Missing the virus is only counted if it gets through the last tube, giving the player multiple chances to vanquish it.

Sick Beats (バイキンはかせ?, Baikin Hakase), also known as Baikin Hakase SP (バイキンはかせSP?, Baikin Hakase SP) in Rhythm Tengoku, is the 2nd Endless Game in Rhythm Tengoku, unlocked with 10 Medals, and the 18th Rhythm Game seen at The Second Gate and can be unlocked as an Endless Game by beating the Gatekeeper Trio's trials in Rhythm Heaven Megamix.
The game is an endless version of the original, ending once a single virus gets through to the end without being stabbed. The game gets faster as it progresses, and new variations of viruses are introduced. Pink viruses take two hits, and black virus take three hits. Each time a virus is hit, the yellow organism releases a variety of objects, with a larger quantity and variety of objects as the game goes on. In Rhythm Tengoku, each successful stab awards the player with 200 points while each mistimed stab awards 100 points, with the maximum possible score being 999900[1]. In Rhythm Heaven Megamix, each stab awards 1 point, up to a maximum of 999 points.
Trials
The player has the option to select which difficulty they want in Rhythm Heaven Megamix.
- Saffron Trial: Only blue viruses are in this version. There are 3 organisms.
- Saltwater Trial: The blue viruses may skip a section. Pink viruses are added. There are 2 organisms.
- Paprika Trial: The blue viruses may skip 1-3 sections. Pink viruses may skip 1-2 sections. Black viruses are added. There are 2 organisms.

- The music used in the Endless Game in Rhythm Tengoku is a slightly remixed version of the normal music. In Rhythm Heaven Megamix, the music is an arrangement in the same style as the original, except for the addition of an ending note in the Gatekeeper Trio's Trials.
- This is the only Endless Game in Rhythm Heaven Megamix whose Trial goal isn't to reach a certain score, but rather to reach the end. The trials last two loops of the game's music, just as long as the Rhythm Game from Rhythm Tengoku. However, the arrangement of the viruses doesn't match it in any of the trials.
- This is one of the few games that doesn't feature a Practice in Rhythm Tengoku.
- It was given a How to Play demonstration in the arcade version and a Practice in Rhythm Heaven Megamix.
- This is the only Rhythm Game in Rhythm Heaven Megamix to return as its Endless Game instead of the normal Rhythm Game. Quiz Show, Shoot'-Em-Up and Bossa Nova also have Endless Games, none of which appear in Rhythm Heaven Megamix.
