Gameplay is the same as before, but the game is extended with increased tempo, requiring the player to peck beaks and stretch necks quickly. There are also many consecutive pecks. The game takes place in a snowy campground, with The Blue Birds wearing coonskin caps while Captain Blue Bird wears a parka.
Perfect!/Ace!?: The player pecks or stretches in sync with the others. While pecking, a hit spark with stars appears. After a stretch, the captain blushes while stretching out his own wings.
Early!/Late!?: The player pecks out of sync, causing less impressive hit sparks to fly out, a "doink" sound to be heard, and a sweat drop to form. While stretching, the player's neck gets a cramp. The captain gets angry and huffs. In the international version of Rhythm Heaven, the animation for mistiming a stretch instead has the bird pose awkwardly.
Miss...?: The player either does nothing, or pecks when they're not supposed to, causing less impressive hit sparks to fly out, a "doink" sound to be heard, and a sweat drop to form. While stretching, they either do nothing or stretch when they're not supposed to. The captain gets angry and huffs.

The Captain has a higher-pitched voice in this game than he does in the first Blue Birds. A similar thing can also be heard in Remix 2, where his voice is lower-pitched.
