Coin Toss (コイントス?, Coin Tosu) is the first Endless Game in Rhythm Heaven unlocked with 2 Medals and the 9th minigame in Rhythm Heaven Megamix at The First Gate. In this game, the player tosses a coin that stays in the air for a certain amount of time and must catch it every time it falls. The background music is simply drums in Rhythm Heaven, but in Rhythm Heaven Megamix it is a laugh track.
Gameplay
The game keeps score by counting every time the player successfully catches the coin. The player must make a judgement on when the coin will land by relying on the rhythm being played in the background. They must count to seven each time. However, they can also use the timer in the first few rounds to aid the timing, but it disappears after said rounds. The background beat will cut off shorter after each catch but still relying on the timing. After the first few rounds of catching, the music will play fully but slower the next time and then repeat that cycle. In Rhythm Heaven, the player's score is reset every time a catch is failed.
Difficulty
The player has the option to select which difficulty they want in Rhythm Heaven Megamix.

Timing Display
- Perfect!/Ace!: You catch the coin.
- Early!/Late!: N/A
- Miss...: The coin is not caught. Your points are reduced to zero in Rhythm Heaven. One miss ends the game in Rhythm Heaven Megamix, if it is the endless version. One miss when you have only one life ends the game if it is the challenge version.

Trivia
- This is one of the few games to require player input before the music starts. Others include Rap Women, DJ School, Clap Trap, Remix 5 (Wii), and Figure Fighter 3.
- This is the only Endless Game in Rhythm Heaven that doesn't have a "Game Over". Instead, it simply resets the player's score. It does have unused sprites for a Game Over screen, however.
- In Rhythm Heaven Megamix, all of the Endless Games have an unused description intended to be seen at the Museum. All of them except for Sick Beats are the same as that of the Saffron Trial. Coin Toss also has an unused title meant for the Endless version ("Coin Toss: Endless").
