The gameplay is identical to its prequel, but the game is extended with increased tempo, requiring the player to peck beaks and stretch necks quickly. There are additionally a large number of overlapping cues, with Captain Blue Bird telling the Blue Birds to peck their beaks while they are already pecking. The Blue Birds now wear ushankas while Captain Blue Bird wears a parka, and the game takes place in a snowy campground.

*Karate Joe and Stomp Farmer make cameo appearances in this game through the flashbacks.
*Similarly to its predecessor, missing any of the last 7 "Stretch out your neck!" cues will instantly give you an OK, and missing 2 or more of the last cues will give you a Try Again, even if you didn't miss anything before that.
*The Captain has a higher-pitched voice in this game than he does in the first Blue Birds. A similar thing can also be heard in Remix 2, where his voice is lower-pitched.
*The Captain has a higher-pitched voice in this game than he does in the first Blue Birds. This is due to his voice only being recorded for the first game, and the game speeding it up to keep up with the faster BPM. This can also be heard in Remix 2, where his voice is lower-pitched.

Blue Birds 2
How can the captain be so foul tempered with such an upbeat song? Stay focused!
Gameplay is the same as before, but the game is extended with increased tempo, requiring the player to peck beaks and stretch necks quickly. There are also many consecutive pecks. The game takes place in a snowy campground, with The Blue Birds wearing coonskin caps while Captain Blue Bird wears a parka.
Perfect!/Ace!?: The player pecks or stretches in sync with the others. While pecking, a hit spark with stars appears. After a stretch, the captain blushes while stretching out his own wings.
Early!/Late!?: The player pecks out of sync, causing less impressive hit sparks to fly out, a "doink" sound to be heard, and a sweat drop to form. While stretching, the player's neck gets a cramp. The captain gets angry and huffs. In the international version of Rhythm Heaven, the animation for mistiming a stretch instead has the bird pose awkwardly.
Miss...?: The player either does nothing, or pecks when they're not supposed to, causing less impressive hit sparks to fly out, a "doink" sound to be heard, and a sweat drop to form. While stretching, they either do nothing or stretch when they're not supposed to. The captain gets angry and huffs.
