Monkey Watch (さる時計?, Saru Tokei) is the 4th Rhythm Game of the second stage (9th overall) in Rhythm Heaven Fever and the 2nd of Shop No. 6 (107th overall), costing 10 Flow Balls, in Rhythm Heaven Megamix.
Gameplay
In order to operate the monkey watch itself, the player controls a small monkey that is holding onto the watch's hand. As the hand is ticking around the clock, other monkeys will come out to greet the main monkey with a high-five to represent the time change on the wrist watch. The monkey hanging onto the clock hand must high-five back. On occasion, two consecutive pink monkeys will appear instead of yellow ones, cued by an "Oookii oook-ook!" sound. These act as offbeats, making the player tap A twice to high-five both monkeys before going back to the regular beat.
The clock hand makes two-and-a-half rotations around the watch in the duration of the song. As it progresses, the games obscures the visuals; quarter-way through the second and third rotation, the screen zooms out to show the whole watch on a man's arm (also showing a monkey face in the middle of it), making it much harder to see the monkeys. After the first zoom-out, a monkey in a balloon will start flying around waving at the screen; this can completely cover up the monkeys as you try to high-five them.

Timing Display
- Perfect!/Ace!: The clock hand monkey will successfully high-five the watch monkeys, making them cheer.
- Early!/Late!: The clock hand monkey will only slightly hive-five the watch monkeys. The regular monkeys will have a frown on their face, while pink monkeys will have a slight smile. A barely counts as a miss in this game.
- Miss...: The clock hand monkey fails to high-five the watch monkeys, making them hang their heads in shame. The man's arm will shake if the screen is zoomed out. The monkey face on the clock makes a "hurt" expression.

- Monkey Watch gets referenced in the gift "A Musical Term" (gotten by perfecting Karate Man 2 (Wii)), where Mr. Upbeat is talking to one (as evidenced by an "Oookii oook-ook!" sound) about his problems.
- Monkey Watch is also mentioned in the Endless Game Bossa Nova at one point in the European version.
- The pink monkeys that serve as off-beats have the appearance of MAN-K, the monkeys that replace the original orange monkeys in Tap Trial's sequel, Tap Trial 2, in both Rhythm Tengoku and Rhythm Heaven Megamix in the Right-Hand Tower as well.
- Monkey Watch is one of the only two games with their songs imported to Super Smash Bros. For Nintendo 3DS/Wii U in the Miiverse stage, the other being Blue Birds from Rhythm Heaven.
- This game has a similar rhythm to Packing Pests.
- The balloon the monkey rides is seen in two other minigames, those being Pajama Party and Jungle Gymnast.
- In the Megamix version, the particles that come out when the monkeys high-five are stars instead of smack symbols. Due to this, the offbeat monkey’s particles no longer fall off-screen. The hot air balloon no longer covers up the offbeat monkeys either.
- There’s an unused sprite of a pink version of the player’s monkey. However, it only has a single recolored sprite of the normal yellow monkey’s head.
- This is one of the few games in the franchise to require player input after the music finishes. Other games like this are Remix 4 (GBA), Remix 5 (GBA), Air Batter 2, Lockstep, Fan Club 2, Lockstep 2, Air Rally, and Flock Step.
