The player controls Karate Joe, and must punch any pot that flies by. Occasionally, there will be a voice saying "Hit 3" along with text, and the player would have to hit two pots in a row, followed by a rock-like object. At certain parts of the song, a "!!" mark will appear by Karate Joe, signaling an item on the offbeat. There is also a combo, which will appear after a light-bulb appears instead of a pot. Directly after the light-bulb will follow with several pots and then a barrel, which the player will have to hit with a flurry of punches and kicks, ending in an uppercut.
As with previous Karate Man games, the player controls Karate Joe, punching any object that flies by. Upon hearing "Hit 3!", the player must hit two consecutive pots followed by a rock. Sometimes, a "!!" mark appears, signaling an object on the offbeat. The main feature is the "Combo!", preceded by a Light Bulb. It consists of several pots and then a barrel, which the player has to hit with a flurry of punches and kicks, ending in an uppercut. The following objects appear and needed to be punched off-screen:

*Pot
In Rhythm Heaven Megamix, the lightbulbs now glow blue (taken from the gameplay in Karate Man Senior), and the game is colored reddish-orange like its sequel, with Karate Man Kicks! using the blue coloration instead.
*Rock

*Soccer Ball
*Light Bulb, followed by a Combo
In Rhythm Heaven Fever, the Light Bulbs do not glow, and the game is colored blue with snow falling, much like the Karate Man in Rhythm Heaven. In Rhythm Heaven Megamix, the Light Bulbs now glow blue (taken from the gameplay in Karate Man Senior), and the game is colored reddish-orange like Karate Man 2 with rising particles, to contrast Karate Man Kicks! having the blue coloration with snow and yellow Light Bulbs.

Two-Player
Two Player
A version of the game can be unlocked in the two-player mode. It is the 8th and final game for that mode, requiring you to beat the original game and the previous two-player game, Tap Troupe, to unlock.

The game has two Karate Joes, standing back-to-back. The first player controls the blue-skinned Joe on the left in a white robe, and the second player controls the purple-skinned Joe on the right with a purple robe. Both players must hit the various objects as they fly out. At a few points, one player goes to the front and is the only one to hit objects.

*This is the third time a Karate Man game has appeared in the Rhythm Heaven franchise, with the other two in previous games.
|explainUK=Karate Joe works hard to<br>hit every combo. It takes<br>fists of steel to become<br>the world's number one!}}
*Just like in Rhythm Heaven, Karate Man is the last "regular" game in Rhythm Heaven Fever before the credits, and the last non-sequel, non-remix game in the entire game.
{{RhythmGame
*The two-player version of the game is the only time that the players don't go by the standard two-player color scheme (P1 = Blue, P2 = Orange).

* This is the only Rhythm Heaven Fever game to have its title card fully changed in the international version of Rhythm Heaven Megamix. The Japanese version includes the Fever title card with the red background.
|image=<tabber>Wii=</tabber>
* In Rhythm Heaven Megamix, this game uses a red color palette despite being the first version of the original. In Rhythm Heaven Fever, the first version is blue and the second version is red instead. This change may be because of the color palette for Karate Man Kicks! being blue already.

Karate Man is the 8th and last Rhythm Game in the Two-Player Menu in Rhythm Heaven Fever. It is unlocked after completing the One Player version.

The game has two Karate Joes, standing back-to-back against a red background. P1 controls the blue-skinned Joe on the left in a white robe, and P2 controls the purple-skinned Joe on the right with a purple robe, being the only Two Player game to not use the cyan/orange color scheme. Both players must hit the various objects as they fly out. At a few points, one player goes to the front and is the only one to hit objects. Unlike the regular game, no particles are shown flying on screen.<gallery>Screenshot Wii Karate Man Two Player.png</gallery>clear
