You are controlling the Clap Trap Doll, who needs your help to protect it from an incoming object. To do this, the player must clap its hands at just the right moment to grab the object about to hit it on the head. Attacks are signaled by three beats, then the player must pause for a beat and then clap. If successful, the attack will be dodged and the game will continue. If failed, the object will hit the doll on the the head and create a bump. If this happens three times, the game will end. The game will constantly change tempo or pause for long periods in-between attacks to throw off the player. The objects that will attack the doll include a hand, a cat's paw, a tree branch with a monkey on it, and a green onion.
In Rhythm Heaven Fever, the gameplay is the same but instead requires two players. The first player controls the doll's right hand while the second player controls its left. In order to block an object, both players must lift their hand at the same time. If only one player does, it will count as a miss.

*In Megamix, there is an expression added where a Clap Trap Doll barely grabs an object. However in Fever, It doesn't make that expression at all. | * In Megamix, there is an expression added where a Clap Trap Doll barely grabs an object.
