The game starts after a whistle. You are a Paddler who is playing a table tennis match with their rival. Your rival will serve a ball to you, & you need to hit it back. There are three speeds of served balls: The normal ball, which goes normally as it bounces smoothly; the slow one, goes a little slower as it bounces higher; and the fast one. Before each fast one, there is a cowbell sound before it to warn you that it's coming at you in fast speed, so hit it as soon as possible. Each cue goes at the same basic speed, but there are different intervals and mixes of them.
The game takes place in a featureless white plain in Rhythm Heaven, and a giant cake in Rhythm Heaven Megamix. The player takes control of a Paddler, who is playing a table tennis match with their rival. The rival serves a ball, and the player must hit it back. There are three speeds of served balls: The normal ball, which bounces on every beat; the slow one, which bounces higher on every two beats; and the fast one, which is preceeded by a cowbell noise, after which the ball is bounces on each half beat before it's hit back.

*This is one of the few games to use 3D models in the Rhythm Heaven Series.
*This is one of the few games to use 3D models in the Rhythm Heaven Series.
*Both this minigame and Rhythm Rally have the same superb message "Nothing beats a good rally!"
*The prologue in Rhythm Tengoku Gold is colored, unlike other regions which show a silhouette instead. This applies to the arrange version as well. There is also an unused version of the prologue featuring a muscular Paddler.
*The Japanese version title is different, as it shows the character rather than the silhouette and uses the letters to show a bouncing ball.
**This is also the same for the sequel.
**The Korean version's title is the same as international version, but with no borders.
