The game remains mostly the same. However, there are minor differences with scoring, and other added features.
The most notable change is in the scoring: the player receives one point for every step rather than every set. In addition, the beat will restart every 32 steps, not every 16.
Every tempo change, Mr. Upbeat will say something to you. At first, he is congratulating you for doing well, but as the game gets faster, he starts wondering how you're able to do this. Eventually, he starts begging you to stop for the sake of his aching feet.

Mr. Upbeat Returns as a bonus minigame in the NA and EU versions of Rhythm Heaven Fever. Though the main premise is the same, there are a few notable differences from its original incarnation:
* The most noticeable change is that the game now counts one point for every successful beat performed, rather than one point for every 16 steps.
* In addition, the Tempo also changes every 32 steps rather than every 16.
* The HUD above Mr. Upbeat's head is now missing.
* The voice-over counting down the beginning of each new tempo change has a feminine voice rather than a masculine one.
* The screen appears to be far more zoomed out than before.
Mr. Upbeat also appears in the [[Endless Remix]].
