The player controls a person's hand holding a fork in the front of a wide blank table. On the other side of the table, a hand is shown through a zooming bubble. When the game starts, the hand will flick pieces of food (mostly pea-like objects with a mini-hamburger at one point) towards the fork. The player will have to get the right timing and spear the food on the fork as it zooms towards it. If food is successfully caught, it stays on the fork until three or four pieces are on it, in which the hand will lift and the person will eat the food. The speed the peas move at is always the same, but how many and the spacing between them varies.

A version of the game can be unlocked for the two-player mode. It is the first game in the mode, and is unlocked along with the two-player menu by beating the original game. The game has the first player using a blue fork, and the second using an orange fork. The hand will flick food at both of the players' forks, requiring them to stab the food. For most of the game, the food will be flicked at the same time by the hand, and the players can stab at the same time. Later in the game, the hand will flick to only one player at a time, with the unneeded fork going off the screen.

*The name "Fork Lifter" is an obvious pun to a forklift operator.
*According to debug notes found in the files of ''[[Rhythm Heaven Megamix]]'', this game, among others, would have a short variant in that game but got cut for unknown reasons.
*This game has slightly been upgraded in ''[[Rhythm Heaven Megamix]]'', in which there are outlines added in the table, while the original didn't have those at all.
*Unlike real-life forks (and more like a trident), the fork has 3 pongs instead of 4. This might be done to make the visual cues better.
*The version in Rhythm Heaven Megamix adds lines to the table for unknown reasons, presumably so players can judge distempered better.


The two players are tasked with catching speeding beans each with a fork. P1 has a blue fork on the left and P2 has an orange fork on the right. The hand shooting beans is now in the center and shoots two beans at a time to both players. At some points, one of the forks moves off-screen, leaving only one player to catch beans.
